I understand if I have any medical, physical or mental issues and or need a medical certificate to join TenCount boxing gym CIO= (TCBGC), it is my responsibility to advise TCBGC Staff and get confirmation in writing from TCBGC before I engage in any activity at the TCBGC .

I also understand I am responsible for advising TCBGC if my medical circumstances change during my membership term.

I understand and expressly agree that my use of any TCBGC facility involves the risk of injury to me or my guest or children under my care whether caused by me or not.

I understand that these risks can range from minor injuries to major injuries including death. In consideration of my participation in the activities and use of the facilities, exercise equipment, or services offered by the TCBGC , I understand and voluntarily accept full responsibility on my behalf and on my guest's or child's behalf for the risk of injury or loss arising out of or related to my use or my guest's or child's use of the facilities, exercise equipment, or participation in exercise programs, techniques taught by TCBGC instructors or other services.
I further agree that TCBGC, its affiliated companies and their respective officers, trustees, employees, members, agents and independent contractors will not be liable for any injury; including, without limitation, personal, bodily, or mental injury, disability, death, economic loss, or any damage to me, my spouse or domestic partner, guests, unborn children, heirs, or relatives, resulting from the negligent conduct or omission of TCBGC or anyone acting on the TCBGC behalf whether related to exercise or not.

Accordingly, to the fullest extent permitted by law, I do hereby forever release, waive and discharge the TCBGC from any and all claims, demands, injuries, damages, actions or causes of action against the TCBGC .
I further understand and acknowledge that the TCBGC does not manufacture fitness or other equipment in its facilities, but purchases and/or leases equipment; therefore, TCBGC will not be held liable for defective products.
I understand any minors under the age of 16 who accompany me whilst visiting TCBGC will be my responsibility.
All minors ages 4 years to 10 years must be accompanied by an adult at all times while using TCBGC .
If I am voluntarily allowing any minors who accompany me whilst visiting TCBGC to participate in activities or gym use, I will assume responsibility for any risk and/or injury that may result due to his/her/their NOT following instructions from TCBGC .

I also understand that said minors will conduct himself/herself/themselves in a responsible manner and treat the facility, its members and staff appropriately.

Failure to comply with the rules and regulations of TCBGC or the terms of this agreement will result in the immediate termination of his/her/their membership.

I agree to waive any claims or rights I might otherwise have to sue TCBGC, its owners, Trustees, employees or other related agents.

I can confirm that:

I have not received a positive test in the last seven days

I am not waiting on a Coronavirus test result

No member of my household has COVID-19 symptoms or is waiting on a test result

To the best of my knowledge I have not been in close contact with anyone who is exhibiting Coronavirus symptoms or has tested positive.

I also agree to maintain social distancing and to keep myself, fellow members and TenCount CIO staff safe by adhering to the safety advice and guidance given.
I understand I can not come to TCBGC under the influence of alcohol or drugs and if suspected TCBGC has the right to refuse my entry and no refund will be given.
